The world's oldest known musical instrument is a flute-like bone carving with 4 holes. The holes are considered to correspond to a portion of a diatonic scale similar to the one we use today. According to archaeologists, it is about 50,000 years old and was supposedly used by Neanderthals. Or at least Neanderthals inhabited the cave it was found in- "Cave Bear".
